We look forward to seeing you soon! Please feel free to print and fill out any of the forms prior to your visit to expedite your check-in.
New Patients
Please complete the following forms and bring them with you to your first visit along with your insurance card, identification, any previous medical records and current medication list. Contact our office to send your immunization record ahead of time. This will allow us to enter your child's vaccine record into the Georgia system and save time at your checkup.
Sports Physicals
We are able to complete the Pre-participation Form for sports as part of your child's annual checkup. Please fill out the "History" section of this form and bring the entire form with you to your appointment.
ADHD Evaluations
In order to accurately assess focus and inattention in children, we need to have assessments from parents and teachers. Please see the attached forms and bring the completed forms to your evaluation.
Insurance Information
We accept Georgia Medicaid and Medicaid CMOs including Wellcare, Peach State, and Amerigroup health plans.
We also accept most private insurance. Please call our office or check with your insurance provider to confirm coverage. We do not accept Kaiser insurance plans.
